What is the Direct Pay Model?The goal of Evolution Physical Therapy is to provide high quality one-on-one physical therapy with a holistic approach. This type of effective, efficient and empathetic care cannot be rushed, limited or capped. Your health goals should not be dictated by your insurance company. ​ During each visit, we strive for quality and value for the patient. We can achieve this level of personalized attention only by being an out-of-network provider. We have the time to fully assess your condition(s), provide hands-on treatment, education, and exercise prescription. We do not utilize support staff such as PT aides, assistants or trainers. ​ We have provided a helpful worksheetfor communicating with your insurance company.

Cancellation PolicyPlease give at least 24-hour notice if you are unable to keep your appointment for any reason. Failure to give 24-hour notice will result in a Cancellation Fee charged to your credit card in the FULL AMOUNT of the appointment price. ​ No-shows for an appointment will result in the same Cancellation Fee. The Cancellation Fee can be avoided by rescheduling your appointment within the same calendar week, if an appointment time is available.

How much does a visit cost?Physical Therapy Evaluation | 75 Minutes | $220 Physical Therapy Follow-up Visit | 60 Minutes | $170 Virtual Physical Therapy Visit | Contact for More Information ​ During each visit, we strive for quality and value for you. What are the office values? Is this office LGBTQ+ friendly?This office appreciates the diversity of human beings and does not discriminate based on race, age, religion, ability, marital status, sexual orientation, sex or gender identity. ​ This office includes a small waiting room, a private treatment room (not an open gym-type environment) and a gender-neutral bathroom. Both the office and building are accessible to people with disabilities. Jessie has completed LGBTQIA ally training through Elon University and several modules published by the National LGBT Health Education Center. She has completed training in providing trauma-informed care. Evolution Physical Therapy & Wellness is part of the community resources project of the LGBT Center of Raleigh.
